WAIVER: MUST BE SIGNED BY PARENT/LEGAL GUARDIAN

I understand that participation in the Tri Kappa Kids Triathlon is a physically demanding activity for any age and that the nature of this event is such that physical injury is a possibility. I hereby declare that the participant is in good health and is able to participate without restriction due to physical infirmity.
I, the undersigned, on behalf of my participant child, myself, my executors, heirs, and administrators, HEREBY WAIVE, RELEASE AND DISCHARGE, the City of Auburn, the Mayor of Auburn, the Common Council of Auburn, the Auburn Board of Public Works and Safety, Auburn Police Department, the Auburn Parks & Recreation Department, YMCA of DeKalb County, Inc., All Star Soccer Academy, Inc. (DBA Rangers Academy), Kappa Kappa Kappa, Inc., Kappa Kappa Kappa, Inc. Alpha Pi Chapter and any agent, employee, member or volunteer of said groups of any claims or liabilities for death, personal injury, or property damage whether or not attributable to the negligence of the parties mentioned above, which arise out of my child’s participation in the Tri Kappa Kids’ Triathlon 2019. I also give my consent that any photos taken which may include my child may be used for future promotional purposes including but not limited to internet, social media, newspaper, or any other print material.
I HEREBY AGREE NOT TO SUE any of the persons or entities mentioned above for any claims or liabilities that I have waived. I HEREBY AFFIRM THAT I AM EIGHTEEN (18) YEARS OF AGE OR OLDER, I HAVE READ THIS DOCUMENT, AND HAVE THE LEGAL AUTHORITY TO SIGN THIS DOCUMENT ON BEHALF OF THOSE LISTED ABOVE, AND UNDERSTAND ITS CONTENTS.
