By my signature, I hereby express my desire to participate in the Operation Snowball Family 5K Run/Walk, sponsored by East Leyden and Elmwood Park High School Operation Snowball Chapters. Please accept my entry and permit me to participate in the race, and in consideration thereof: 1) I hereby represent that I understand that participating in this run or walk is physically strenuous and that I am physically able to participate and compete. 2) I hereby for myself, my heirs, executors and administrators or assigns, waive and release any and all rights and claims for damages which I may have against the East Leyden and Elmwood Park High School Operation Snowball Chapters, East Leyden and Elmwood Park High Schools, Unit School District 212 and 401, the Villages of Northlake, Melrose Park, Franklin Park, Elmwood Park, River Grove, Schiller Park and Rosemont and other their representatives, successors, agents, employees or assigns, volunteers or workers and sponsoring individuals or organizations for any and all injuries and or damages I may suffer as a result of participation in this event. I also waive notice and grant permission for free use of my name and or picture in publications or other accounts of this race. NO ENTRY WILL BE ACCEPTED WITHOUT ACKNOWLEDGEMENT BY SIGNATURE AND RECEIPT OF ENTRY FEE
