By my signature, I hereby express my desire to participate in the Operation Snowball Shuffle Family 5K Run/Walk, sponsored by Elmwood Park High School Operation Snowball Chapter. Please accept my entry and permit me to participate in the race, and in consideration thereof: 1) I hereby represent that I understand that participating in this run or walk is physically strenuous and that I am physically able to participate and compete. 2) I hereby for myself, my heirs, executors, and administrators or assigns, waive and release any and all rights and claims for damages which I may have against the Elmwood Park High School Operation Snowball Chapter, Elmwood Park High School, Unit School District 401, the Villages of Elmwood Park, River Grove and other representatives, successors, agents, employees or assigns, volunteers or workers and sponsoring individuals or organizations for any and all injuries and or damages I may suffer as a result of participation in this event. I also waive notice and grant permission for free use of my name and/or picture in publications or other accounts of this race. NO ENTRY WILL BE ACCEPTED WITHOUT ACKNOWLEDGEMENT BY SIGNATURE AND RECEIPT OF ENTRY FEE
